As a sponsored walker, you will want a Pledge Form to record sponsors paying by cash, check or wanting to be billed by Alternatives. We have provided the Pledge Form for you online as a PDF document. Simply click here to download the Pledge Form.

Please have each of your sponsors complete all the information on the Pledge Form. Then bring your Pledge Form with you on the day of the Walk. Remember to let your sponsors know that their donation is tax-deductible.
